Our Care Programs
Our programs are designed to provide affordable alternative support to a broad category of individuals who prefer to live in the community including those 65 and older who qualify to live in a nursing (Alternative Care waiver), seniors who qualify to live in a nursing home and are eligible for Medical Assistance (Elderly Waiver), those who are chronically ill and require hospital level of care (Community Alternative Care waiver), those who have a disability and require nursing facility level care (Community Access for Disability Inclusion Waiver), those with disabilities or related conditions who qualify to receive care in an Intermediate Care facility for Persons with Developmental Disability (ICF/DD)(Developmental Disability waiver) and those diagnosed with an acquired or traumatic brain injury who qualify for help in a nursing facility or a neurobehavioral hospital (Brain Injury Waiver).
