Services We Offer
Our services target a broad category of individuals who prefer to live in the community under different waiver programs or private pay. This includes seniors (65 and older) who qualify to live in a nursing homes, people who are chronically ill and require hospital level of care, people who have a disability and require nursing facility level care, people with disabilities or related conditions who qualify to receive care in an Intermediate Care facility for Persons with Developmental Disability, and for people diagnosed with an acquired or traumatic brain injury who qualify for help in a nursing facility or a neurobehavioral hospital.

We provide Individualized need-based ongoing CBS to persons with intellectual/neurodevelopmental disability, Traumatic Brain Injury, Seniors (65+) & chronically ill persons, in place of institutions or nursing homes and hospitals. Our services are designed to maintain or improve physical, intellectual, emotional, and social function.
WITH COMMUNITY-CONNECT HOME CARE I AM CONNECTED
Options for adults & seniors who choose living in their home and participate in community instead of living in a nursing home or prolonged hospital stay. Our services are designed to promote social inclusion.
Our services include basic levels of assistance, supervision, and care that is necessary to ensure the health and welfare of the person and professional nursing services based on an assessment of the person’s medical/health care needs like care provided in hospitals.
- Home Health Services (Certified Nursing Assistant, Home Health Aide, LPN, RN & Skilled Nursing Visit)
- Medication management services
- Delegated tasks to unlicensed personnel
- Hands-on assistance with transfers and mobility
- Eating assistance for clients with complicating eating problems (i.e. difficulty swallowing, recurrent lung aspirations, or requiring the use of a tube, parenteral or intravenous instruments)
- Assistance with dressing, self-feeding, oral hygiene, hair care, grooming, toileting, and bathing
- Standby assistance within arm’s reach for safety while performing daily activities
- Verbal or visual reminders to take regularly scheduled medication (includes bringing clients previously set-up medication, medication in original containers, or liquid or food to accompany the medication)
- Verbal or visual reminders to the client to perform regularly scheduled treatments and exercises
- Preparing modified diets ordered by a licensed health professional
- Laundry
- Housekeeping/other household chores
- Meal preparation
- Shopping
- Medication management
WE CONNECT INDIVIDUALS TO COMMUNITIES
We provide dependable and timely non-emergency medical transportation (NEMT) services to ensure individuals can access essential medical appointments, therapies, and other healthcare services. Our trained drivers prioritize safety and comfort, offering a seamless experience for those who require assistance getting to and from their destinations. Please note, this service is not reimbursable or billable for individuals in our residential settings.
