We provide Individualized need based ongoing residential support with training or rehabilitation to persons with intellectual/neurodevelopmental disability, challenging behaviors & Traumatic Brain Injury in a Community Residential setting (CRS) in place of institutions or nursing homes and hospitals.
WITH COMMUNITY-CONNECT HOME CARE I AM CONNECTED
Options for adults & seniors who choose living in their home and participate in community instead of living in a nursing home or prolonged hospital stay. Our services are designed to promote social inclusion.
Our services include basic levels of assistance, supervision, and care that is necessary to ensure the health and welfare of the person and professional nursing services based on an assessment of the person’s medical/health care needs like care provided in hospitals.
WE CONNECT TO COORDINATE SERVICES
We provide well designed positive support & crisis respite services to manage challenging behaviors, depression and anxiety. Our mental health services target individuals including People who are neuro-divergent, those experiencing Co-occurring aging, disability and/or mental health concerns, People with dementia /memory loss & People with traumatic brain injury (TBI).
WE CONNECT INDIVIDUALS TO COMMUNITIES
We provide dependable and timely non-emergency medical transportation (NEMT) services to ensure individuals can access essential medical appointments, therapies, and other healthcare services. Our trained drivers prioritize safety and comfort, offering a seamless experience for those who require assistance getting to and from their destinations. Please note, this service is not reimbursable or billable for individuals in our residential settings.
