Position Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our growing team. The LPN will provide direct client care, support nursing delegation, assist with medication management, and ensure compliance with Minnesota Department of Health (MDH) and 245D regulations. This role blends clinical expertise with compassionate caregiving, ensuring that each client receives the highest quality of care.

Key Responsibilities

  • Deliver hands-on nursing care, including medication administration, treatments, and monitoring of client health status.

  • Assist with the development and implementation of individualized care plans in coordination with the Nurse Manager and RN staff.

  • Conduct competency evaluations for unlicensed personnel and provide ongoing coaching to care staff.

  • Maintain accurate, timely documentation in compliance with state regulations and company policies.

  • Participate in care team meetings, orientation, and ongoing staff training (including Alora and EduCare systems).

  • Support clients and families with education and resources, ensuring dignity, safety, and respect in all interactions.

  • Respond promptly to client changes in condition and emergencies, following established protocols.

  • Model Community-Cannect’s values by being a caregiver first, even when working in administrative or supervisory capacities.

Qualifications

  • Current and active LPN license in the state of Minnesota.

  • CPR/First Aid certification (required).

  • TB screening and up-to-date immunizations.

  • Previous experience in home care, long-term care, or community-based services preferred.

  • Strong communication, organizational, and interpersonal skills.
