In-Home Care Overview: Community – Connect Home Care
I was once faced with discrimination and isolation but found hope in the kindness of a lone friend, Yuka, whose warmth and companionship changed my life. Tragically, Yuka later passed away due to challenges related to his disability and mental health. In his memory, I committed myself to serving people with disabilities and mental health challenges—a commitment that continues to this day.
In search of extra income, I accepted a job in an assisted living and dementia care unit. What began as a financial decision became a life-changing experience. I discovered a deep passion for working with seniors and an innate ability to connect with them. I found beauty in aging, meaning in comforting the dying, and purpose in understanding the needs of those often overlooked.
But I also witnessed the many challenges seniors face: frequent falls, missed medications, delays in receiving needed therapy—problems made worse by a fragmented healthcare system. This inspired me to imagine a better way of supporting both seniors and people with disabilities. That vision began to take shape when one resident, who had decided to return home, insisted I continue caring for her there. That moment sparked what would become Community-Connect Home Care LLC—a service dedicated to helping many more people just like her.
Years of work in nursing homes, assisted living facilities, and group homes—supporting individuals in crisis, with disabilities, and mental health challenges—fueled my decision to pursue a Doctor of Pharmacy (PharmD) degree. As a community pharmacist today, I’m even more aware of how difficult it is for vulnerable individuals to access the care, medications, and support they deserve.
