Community Based Services (CBS) & Community Residential Services (CRS)
provide Individualized need based ongoing residential support with training or rehabilitation to persons with intellectual/neurodevelopmental disability, challenging behaviors & Traumatic Brain Injury in a Community Residential setting (CRS) in place of institutions or nursing homes and hospitals. Community Alternative Care (CAC) Waiver
CAC is provided for chronically ill or medically fragile people who would otherwise be in the hospital. “The services help a person live as independently as possible in community settings and promote optimal health, independence, safety and community integration” MDH.
Community Access for Disability Inclusion (CADI) Waiver
A community-based service option for people with disabilities who qualify for care provided in a nursing facility. These services are designed to help a person live as independently as possible in community settings and promote optimal health, independence, safety, and community integration.
Developmental Disability (DD) Waiver
A community-based service option offered to people diagnosed with DD who qualifies to receive care in an intermediate care facility for persons with developmental disabilities (ICF/DD). The purpose is to help a person live as independently as possible in community settings and promote optimal health, independence, safety and community integration.
Semi-Independent Living Skills (SILS)
Services needed by an adult with a developmental disability or related condition(s) to live successfully in the community.
Integrated Community Supports (ICS)
Services that provide support and training/habilitation in community living service categories to adults age 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multifamily housing building). Services are individualized and based on the person’s assessed areas of needs.
Individualized Home Support (IHS) with Training
Services for adults when they need support and training in at least one of the community living service categories when living in their own home or family home. Training includes habilitation. Community living service categories include community participation; health, safety and wellness; household management; adaptive skills.
