We provide basic levels of assistance, supervision, and professional nursing care that are necessary to ensure the health and welfare of the person, like the care provided in hospitals. The people served remain connected to their hospital care team at a reduced cost and free from the risk of hospital acquired infections as they are served in the comfort of their homes.

Medication Management
Our Medication Management service ensures that clients take their medications correctly and on schedule. Licensed healthcare professionals oversee medication administration, track dosages, and provide education on proper medication use to prevent errors and promote optimal health outcomes.
Personal Care Assistant (PCA)/Certified Nursing Assistant Services
With day-to-day activities in home and in community to help a person maximize their independence. Services include assistance with ADLs (dressing, self-feeding, oral hygiene, hair care, grooming, toileting, and bathing); assistance with home management (laundry, housekeeping/house chores, meal preparation and shopping); Medication reminders, reminders for scheduled treatment/exercise, preparing modified diets ordered by a licensed health professional.

Individual community living support (ICLS)
Designed to support people age 65 and older enrolled in the Alternative Care (AC) program or the Elderly Waiver (EW) who need reminders, cues intermittent/moderate supervision or physical assistance to remain in their own homes. It includes the following service components:
Health, safety and wellness: Health maintenance cueing to ensure the person follows prescribed treatments
Assistance with basic living tasks and social skills.
Help applying for & maintaining benefits
Assistance with household management.
Problem-solving support.
Active cognitive support.
Adaptive support service.Activities of daily living (ADLs) support
Community living engagement.
Adult Companion Care
A non-medical support provided to an adult working to achieve a therapeutic or community integration goal that may include coping skills to manage social anxiety, social isolation or to enhance fine motor skills.
We provide companionship by supervising/accompanying/keeping company with an individual as they work to achieve a goal within their support plan. Attending a movie with the person to practice coping skills to manage their social anxiety. Going with the person to a community event to reduce their social isolation. Play a board game with the person to enhance their fine motor skills. Provide verbal instructions or cues to the person to help them complete a task. Assist or supervise the person with tasks such as laundry, light housekeeping, meal preparation and shopping. Companion care does not include support with ADLs.
Individualized home support without training
personal support Services for adults or children when they need support, assistance and supervision in at least one of the community living service categories when living in their own home or family home. Community living service categories include community participation, health, safety and wellness, household management & adaptive skills.
Respite Care
Temporary supervision and care necessary to ensure the health and safety of the person due to the absence or need for relief of the primary caregiver.
We ensure the health and safety of the person we support and reassure them of the connection with their loved ones. No training and/or rehabilitation involved. We provide respite care services only in the individual’s own home or family home.
24-Hour Emergency Services
On-call counseling & problem solving and/or immediate response for support at a person’s home due to a health or personal emergency that is not related to an injury or an immediate danger requiring 911 assistance.
Assistance provided is specific to the person’s needs, as per their support plan.
Night Supervision
Provides overnight assistance and monitoring by staff in the person’s own home. This may include support for better sleep hygiene and safety checks.
