Mental Health Care/Complex needs & Challenging Behaviors

We provide well designed positive support & crisis respite services to manage challenging behaviors, depression and anxiety. Our mental health services target individuals including People who are neuro-divergent, those experiencing Co-occurring aging, disability and/or mental health concerns. People with dementia /memory loss & People with traumatic brain injury (TBI).

Positive Support

We implore positive support interventions with the least restrictive mode to support individuals with challenging behaviors. Our proactive plan to address challenging behaviors suggests an alternative, positive behavior, that will result in an increase in the person’s quality of life and decrease challenging behavior.
Our positive support plan/or positive support transition plan is professionally designed to enhance a person’s quality of life by coaching, teaching or encouraging to increase positive behavior. These plans are subject to periodic reassessment and modification. Use of person-centered approaches that incorporate a comprehensive, functional behavior assessment of both positive and challenging behavior. Emergency use of manual restraint (EUMR) is a last resort to help prevent incidence that could be life threatening. The use of restrictive interventions may only be temporary and accompanied by a Positive Support Transition Plan.

Crisis respite (Coping skills counselling & Mental Health crisis management)

We provide out-of-home crisis respite in a Community Residential Setting (CRS). We assess the person and situation to determine triggers and provide counseling and coaching and develop support plans to train on coping skills to manage mental health crises.
In crisis respite, we assist, supervise and provide care necessary to ensure the health and welfare of the person.
Develop a person-centered intervention plan, in coordination with the person and support team, that is based on recommendations in the assessment.
Consult with and train the provider(s) and/or caregiver(s) to ensure successful implementation of the intervention plan.
Recommend positive support strategies and revisions to the person’s support plan to prevent or minimize future crisis situations and increase the stability of the person living in the community.
Develop and implement a transition plan to support the person’s return to home if they receive out-of-home crisis respite.

Support for People with Traumatic Brain Injury (TBI)

We provide services to people with TBI in either our residential setting or own home. An option for people diagnosed with a traumatic, acquired, or degenerative BI who receive care in a residential setting that would otherwise be provided in a nursing facility with specialized services for people with BI, or in a neurobehavioral hospital.

Integrated Community Supports (ICLS) for People with Dementia/Memory Loss

Our Integrated Community Supports (ICLS) program is designed for individuals experiencing dementia or memory loss, offering a combination of in-home care, daily living assistance, and structured cognitive activities to promote mental engagement. Services include medication reminders, personal care, safety monitoring, and companionship, ensuring individuals receive the necessary support to maintain their dignity and independence while aging in place.
